- All services are done by appointment only, including new memberships.
- If you wish to have a tour of the facility or apply for new membership, please create an appointment below or call.
- Submit a membership application package prior to your appointment time.
- Receive a discount on your membership with a Physician and this form.
- If you wish to cancel, we are sorry to see you go, complete this online form & follow the closeout instructions.
Appointments for New Memberships & Facility Tour:
*Limited Appointments. Call to schedule if you have special work circumstances or need to begin immediately. Tuesdays & Thursdays are preferred.
Note if you’re more than 10 minutes late, your appointment will be canceled, and you will need to reschedule.
